Population Health Management is a modern approach to addressing health inequalities by taking a whole population approach to improving the management of risks of a given cohort and complements the adoption of integrated care systems by embracing community and non-health related solutions.
Across the developed world, health and social care systems face a series of fundamental challenges. These include ageing populations, rising expectations, lifestyle-related illness, resource constraints and rapid technological change. Population Health Management, with its focus on understanding the needs of the population in a granular way, is an essential part of the response.
Basic Theory of Population Health Management
Active Black Country are currently working with the Black Country STP to develop a PHM project that aims to improve levels of physical literacy amongst young people in our 4 Local Authority areas. Learnings from this exciting piece of work will be available to view on our Insight Hub.
